Licensed Family Child Care Home

What you need to know

  • A Family Child Care Home must have a working, listed telephone number.
  • The program must be in the home of the child care provider.
  • A License becomes void if you move, and you must contact the Child Care Licensing Regional Office for your county.
  • A $15 Licensing Fee is due in January after a License is issued and every two (2) years thereafter.

Application Process

  1. Attend Orientation (Call the Child Care Licensing Regional Office for your county to sign-up for Orientation).
  2. Submit Zoning Approval to the Child Care Licensing Regional Office for your county.
  3. Complete the following forms and mail them to the Child Care Licensing Regional Office for your county. Click the form number to open a copy of the form.
    • Application to Operate a Child Care Facility (DSS Form 2902).
    • Health – Fire Inspection Request (DSS Form 2905).
    • Household Member List (DSS Form 2927).
    • Consent to Release Information Forms (DSS Form 2924) for the operator, all caregivers, and all Household Members over the age of 18. Use Full Names (No Initials) on these forms. Mail these forms with a check or money order ($8.00 per person) made payable to SCDSS to the Child Care Licensing Central Office at the address below. You may also mail the forms to the address below and pay $8.00 per person online by clicking here (make sure you complete the online payment section of the form if you pay online).

  4. Have State and Federal fingerprint checks completed on the operator, all other caregivers, and all household members 15 years and older.Only scanned prints are now accepted. Click here for more information on scanned fingerprints.
  5. DSS Child Care Licensing will conduct an Inspection. Have the following on file for the inspection:
    • First Aid/CPR Certification to cover all hours of operation.
    • Medical Statements (DSS Form 2901) for operator, all other caregivers, and all Household Members.
    • Health Assessments (DSS Form 2926) for operator and all other caregivers.
  6. Training for operators, caregivers, and emergency people must be completed (Required for renewals only). For more information click here.
